 APPOINTMENTS/REGISTRATION

The Practice accepts registrations from members of the public who move into the practice area on presentation of a medical card or completion of a medical registration form.  Patients will be asked to provided two supporting documents to confirm these details one of which should confirm your address is within our catchment area.  If you have just come into the UK we will need proof that you intend to work or study in the UK (a work permit or supporting letter from your employer or the educaiton provider).

New patients are requested to make an appointment with one of our Nurses for a Health Check within 4 weeks of registration.

All consultations are by appointment, which can be made in person or over the telephone.  Appointments can be made up to 6 weeks in advance.  We do our best to respond to patient requests and offer them appointments as quickly as possible.  Please make it clear to the Receptionist if your request is urgent.

We recommend that patients see their registered Doctor whenever possible.  If this proves difficult or there is a particular problem, patients may see one of our other Doctors.

Patients are requested to inform the surgery if they are unable to attend an appointment.

CHANGE OF PERSONAL DETAILS

Please help us by keeping us informed of any change in your personal details i.e. name, address, telephone numbers etc.  Proof of evidence will be required in respect of change of name.  If you move outside of our Practice area you will need to register with another Doctor in your new locality.  Our Receptionist will be able to advise you about this.

ENQUIRIES AND TEST RESULTS

Results of blood tests, X-rays and correspondence are received during the morning and have to be checked by our Doctors.  If you wish to know a result please ring after 1200 hours.  For confidentiality reasons we can only give results to the patient concerned.

COMPLAINTS

The Practice Manager welcomes suggestions from patients which may help enhance or improve the services provided by the surgeries.  If, however, you are dissatisfied in any way we would also like to hear from you.

 PATIENT PARTICIPATION GROUP

The Patient Participation Group exists to help the Practice by representing the views of patients, by assisting in health promotion activites and by encouraging patients to take a more informed interest in their own health care.  The Group meets regularly with the Doctors and the Practice Manager.  The Chair is Julie Grist who can be contacted on 01428 683448 or by email at julie.grist@lineone.net

 

MILFORD AND WITLEY VILLAGE CARE

This is group of local volunteers who help local residents who need assistance.  Most of the help provided is car transport but other tasks are carried out.  If you require the assistance of Village Care, a message can be left for the Co-ordinator on 01483 424393.  New volunteers are always needed!  If you think you would be able to help please contact Katie Wells on 01483 860969.
