How to register

Patients living within the practice area will need to complete registration forms for each member of their family. Please collect forms from either Witley or Milford Surgery or print off from the website.  Completed forms should be handed in to reception.  All adults are welcome to make an appointment with the GP for a New Patient Check. You will need to complete all the forms below before you can be seen by a doctor or nurse.

It will take some time before your medical records arrive at the practice, it would be very helpful if you could provide us with your past and current medical conditions including any medications you may be taking.

For regular repeat medications, you will need to book an appointment with a GP before we can issue your next prescription. At registration you will be offered the GP who has availability to take on new patients at the time. If you prefer another GP your request will be taken into consideration however we cannot guarantee it.


Out of Area Registrations

We understand that some people may wish to join a GP surgery near work or register with their old GP following a move.

We do accept patients outside our normal boundary under this scheme, based on certain conditions and only if it is clinically safe and appropriate to do so.

There are some important things to consider when considering whether to register with a practice that is not local to your home.

Because of the greater distance to your home, we will not be able to provide a home visiting service. For frail patients, or for those with significant long-term conditions who are likely to require care at home, this scheme would not be suitable.

Whilst many medical conditions can be managed at a distance, people on long-term medical treatment are likely to need face to face medication safety reviews from time to time. If it is difficult to get to the practice, then it would be wise to choose a surgery closer to home.

Registration away from home can also sometimes affect decisions about referral for hospital tests and treatment, or access to community health services.  This also needs to be considered when deciding on the best practice for you and your family.

If you wish to consider registering under the Out of Area Scheme, please complete the attached form below and either drop it in to the surgery or send via AskMyGP.  A separate form is required for each person. 

Parents/Guardians must sign on behalf of any child under 13. A parent can sign fort young people aged 14 or 15 or they can sign themselves.  Anyone over 16 must sign the form themselves.

This form will then be passed to our clinical management team who will consider whether it is suitable and safe for you. You will be notified as quickly as possible by your preferred method.
